I am a GP, how do I make a referral?

Under the Better Access to Mental Health Care initiative, a GP can refer a patient for therapy by writing a referral letter accompanied by a Mental Health Treatment Plan (MHTP). You may also consider administering a K10 and DASS.

The referral can be addressed directly to “An Nguyen” or the “Mental Health Professional”

Referral Requirements

A referral should be signed and dated and include:

  • the patient’s name, date of birth and address;
  • the patient’s symptoms or diagnosis and a statement on whether a mental health treatment plan has been prepared;
  • a list of any current medications;
  • the number of sessions the patient is being referred for (usually 6 or 10); and
  • a statement about whether the patient has a mental health treatment plan or a psychiatrist assessment and management plan.

The patient can then contact us to arrange an appointment.

The patient can access to up to 10 sessions of focused psychological strategies per year.

What feedback will I receive?

Medicare requires mental health professionals to provide a written progress report back to the referring GP after 6 sessions of treatment, as well as a final report outlining information on assessments, treatment provided and recommendations for future management.
